If you have had formulas filled through the apothecary before you will need create an account
Fill out the form below and we will register you.
You should receive a verification email from Dispensary Tree where you will be asked to create a username and password - if you do not receive this email, please email us.
Once your account is created, get in touch with your practitioner or email us to add your formulas. You will need to re-order a formula to activate refills - this is just how DT works.
From your new account you can order, pay for formulas & submit refills.
